The aim of the study is to increase the skills of District Nurses (DNs) to manage frailty. The core idea is to create, deliver and evaluate a person-centred intervention with two components: a coaching programme for DNs to strengthen their leadership role in managing frailty within interprofessional teams, and an educational intervention about effective and personalised care and support for people living with frailty in the community. This part of the study is expected to last for 12 months.
